Senior Vista Initial Assessment

What It Is:

This form collects basic information about your loved one's health, daily needs, and care requirements. It helps us understand how we can best support them.

What You'll Need:

- Basic medical history

- Current medications and dosages

- Emergency contact information

- Any dietary restrictions or allergies

- Mobility and assistance needs

Time to Complete: 15-20 minutes

💡 Tip: Have your loved one's medication bottles handy when filling this out.

TB Skin Test

What It Is:
Florida state regulations require proof of a negative TB (tuberculosis) test for all adult day care participants. This form must be completed by a healthcare provider.

What You'll Need:
- Schedule a TB skin test with your loved one's doctor
- Have the doctor complete this form after the test
- Test must be current (within the past 12 months)

Time to Complete: 5 minutes (plus doctor appointment)

💡 Tip: Many pharmacies (CVS, Walgreens) offer TB tests- often faster than scheduling with a doctor.

Tops Application

What It Is:
If your loved one qualifies for Medicaid Long-Term Managed Care (LTMC), this application is required to process coverage through your Medicaid provider.

When You Need This:
- If you're using Medicaid to cover adult day care costs
- If you're unsure if you qualify for Medicaid

Don't Have Medicaid? You can skip this form if you're paying privately or using Long-Term Care Insurance.

Time to Complete: 20-30 minutes

What Else You'll Need

In addition to the forms above, please bring the following to your enrollment meeting:

Insurance Cards

- Medicaid card (if applicable)

- Long-Term Care Insurance policy information

- Medicare card (for reference)

Identification

- Photo ID for your loved one (driver's license or state ID)

- Social Security card or number

Medical Information

- List of current medications (or bring the bottles)

- Doctor's contact information

- Any advance directives or healthcare proxy documents

Emergency Contacts

- Names, phone numbers, and relationship to participant

- At least 2 emergency contacts required

A: No problem. You can enroll first and provide the TB test results within the first week. We'll guide you on where to get tested quickly.

Q: Do I need a doctor's referral?

A: No, you don't need a formal doctor's referral to enroll. We just need basic medical information and the TB test.

Q: What if my loved one is on Medicaid but I don't have all the paperwork?

Q: Can I email completed forms ahead of time

Q: How long does the enrollment process take?

A: Once we have all required documents, most families are enrolled within 1-3 business days. Your loved one can often start within the same week
